Hospice of North Alabama in Scottsboro, AL is seeking a passionate Hospice RN Case Manager to join our team. You will provide expert, patient-centered care at a critical time in people’s lives, working in a home-based setting with a supportive team.

This full-time role offers a stable schedule, Mon-Fri 8am-5pm, with on-call rotation as required, competitive benefits, and opportunities for professional growth within a growing organization.

Qualifications

• Graduate of an accredited nursing school with current state licensure
• 1 year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Possess CPR certification in required states

Responsibilities

• Assess/monitor patients' physical, emotional, and psychological needs
• Create and implement hospice care plans aligned with patient goals
• Direct nursing care: administer medications and treatments
• Provide pain and symptom management
• Educate and support patients' families and caregivers
• Collaborate with interdisciplinary team and document accurately
• Participate in on-call rotation as required by local branch

As a RN Case Manager You will:

• Assess/ monitor physical, emotional, and psychological needs of patients

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
