Job summary

Hospice of North Alabama is seeking an experienced Registered Nurse Case Manager to join our team in Scottsboro, AL. You will assess patient needs, develop care plans, administer nursing care, and manage pain and symptoms with compassionate, patient‑centered support.

Responsibilities include educating families, collaborating with the care team, maintaining documentation, and participating in on-call rotations. Requires AL RN license and CPR certification; hospice experience preferred.

Qualifications

• Graduate of an accredited nursing program with current AL licensure.
• One year nursing experience required; 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of hospice philosophy of care.
• Must maintain CPR certification where required.

• Assess/monitor physical, emotional, and psychosocial needs of patients.
• Create and align hospice care plans with patient goals.
• Direct nursing care including medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients' families and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
