BrightSpring Health Services in Georgia is seeking a qualified Registered Nurse Case Manager to join the hospice team in McDonough area. You will provide patient-centered nursing care, develop care plans, and work with an interdisciplinary team to meet patient goals.

The role requires RN licensure, 1 year of nursing experience (3+ preferred), and a commitment to clinical excellence and compassionate care. Valid driver's license and insured vehicle are required for field visits.

Qualifications

• RN licensure in state of employment.
• Minimum 1 year nursing experience; 3+ years preferred.
• Hospice or equivalent setting experience preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must have a valid driver’s license and insured vehicle.

Responsibilities

• Assess and monitor patients' physical, emotional, and psychological needs.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care, administering medications, treatments, and interventions.
• Manage pain and other symptoms.
• Educate and support the patient's family and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.

Office Location & Coverage Area

McDonough, GA. Care is provided to patients in Spalding, Pike, Lamar, and Butts counties.

Schedule

Monday to Friday, 8 am to 5 pm.
