Job summary

Adoration Health in Lawrenceville, GA is seeking a Registered Nurse Case Manager for hospice care. You will provide one-on-one patient care, develop care plans, and collaborate with an interdisciplinary team.

Based in Lawrenceville, GA, this full-time on-site role offers a Monday–Friday schedule with major/minor holiday call. You will educate families, manage symptoms, and document care with accuracy.

Qualifications

• Graduate of an accredited nursing school with current state licensure.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
