Job summary

Brighton Hospice is seeking a Registered Nurse Case Manager to serve patients across the Atlanta area, including private homes and facilities. This full-time role involves coordinating clinical care, psychosocial assessments, and community resource use, as part of an interdisciplinary team.

Travel within the service area is required. The ideal candidate holds a GA RN license in good standing, with hospice experience and the ability to work independently while collaborating closely with a care

Qualifications

• Current RN license in Georgia (in good standing).
• Ability to collaborate daily with an interdisciplinary team.
• Ability to work closely with patients one-on-one while receiving strong support from the care team.
• Capable of working independently with phone, email, documentation, meetings and hospice regulations.
• Strong time-management to schedule visits, complete documentation, and prioritize urgent needs.
• Willingness to learn and use an electronic medical records system.

Responsibilities

• Provide care for a defined patient group, including home visits and facilities.
• Act as an interdisciplinary team member with expertise in psychosocial assessments and community resources.
• Coordinate patient care across settings to support comfort and quality of life.
• Maintain accurate documentation and communicate needs among care team.

Details

The Registered Nurse Case Manager (RN) will be responsible for providing care and services for a group of patients and visiting patients where they reside, including private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing interdisciplinary team member, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.

Qualifications

• Current Registered Nurse (RN) licensure (in good standing) in the state of Georgia
• Works well within a team environment to collaborate daily with our interdisciplinary team
• Work closely with patients on a 1-1 level but get strong support from a patient care team.
• Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
• Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
• Ability to learn an electronic medical records system.
• Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area
• Previous hospice experience required
