Job summary

Springhill Hospice in Daphne, AL is seeking a dedicated Registered Nurse to join our hospice team as an RN Case Manager. You will provide patient-centered, 1:1 care and collaborate with an interdisciplinary team to plan and deliver comfort-focused care in patients' homes or community settings.

Qualifications

• Graduate of an accredited nursing program with current licensure to practice in Alabama.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Must have a valid driver's license and CPR certification.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
