Job summary

Haven Hospice in Chiefland, FL is seeking a dedicated Registered Nurse Case Manager to provide 1:1 hospice care and coordinate patient plans. The role emphasizes compassionate patient-centered care and collaboration with a multi-disciplinary team.

As an RN Case Manager you will assess and monitor patient needs, develop care plans, administer treatments, and support families. This position requires Florida RN licensure and reliable transportation.

Qualifications

• Graduate of an accredited nursing program with current licensure to practice in the state of operation (Florida).
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
• Must be a licensed driver with reliable automobile.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
