Job summary

Traditional Home Health Care is seeking a Registered Nurse Case Manager to deliver high-quality in-home nursing care in the Waymart, PA area. This full-time role supports patient-centered plans and collaborates with physicians, therapists, and families to ensure optimal outcomes.

You will assess patients, implement care plans, and document progress while coordinating with the healthcare team. A PA RN license and BLS are required, with strong communication and time-management skills valued.

Qualifications

• Active PA RN license required.
• BLS certification required.
• Valid driver’s license with reliable transportation.
• Strong assessment and documentation skills essential.
• Ability to work independently and manage time effectively.
• Excellent communication and interpersonal skills.
• Acute care and/or home health experience preferred.

Responsibilities

• Assess patients and develop personalized care plans with physicians, nursing supervisors, patients, and families.
• Provide hands-on care, administer medications, and evaluate patient progress.
• Maintain and submit timely documentation, including OASIS assessments.
• Collaborate with therapists, caregivers, and healthcare professionals for comprehensive care.
• Educate patients and families on treatment plans and self-care techniques.
• Inform patients of additional services and resources as needed.

Full-Time - Monday through Friday 8:30am to 5:00 pm
