Job summary

Traditional Home Health Care in the Nanticoke area seeks an experienced RN Case Manager to coordinate patient care at home, partnering with physicians, nursing supervisors, families, and therapists. The role emphasizes hands-on nursing, medication administration, and timely documentation to support optimal patient outcomes.

The successful candidate will hold an active PA RN license, BLS, a valid driver's license, and strong communication, assessment, and time-management skills.

Qualifications

• Active RN license in Pennsylvania is required.
• BLS certification is required.
• Valid driver's license and reliable transportation.
• Strong assessment and documentation skills.
• Ability to work independently and manage time effectively.
• Excellent communication and interpersonal skills.

Responsibilities

• Assess patients and develop personalized care plans with physicians, nursing supervisors, patients, and families.
• Provide hands-on care, administer medications, and evaluate patient progress.
• Maintain and submit timely documentation, including OASIS assessments.
• Collaborate with therapists, caregivers, and healthcare professionals for comprehensive care.
• Educate patients and families on treatment plans and self-care techniques.
• Inform patients of additional services and resources as needed.

Schedule: Full-Time -Monday through Friday 8:30am to 5:00pm

Your Role as an RN Case Manager:
