Continuum Hospice & Palliative Care California is seeking a compassionate Hospice Registered Nurse Case Manager (RNCM) to support patients and families through meaningful moments of care in the Santa Cruz and San Jose communities.

This full-time role offers a Monday–Friday schedule, comprehensive benefits, and opportunities for professional growth as you coordinate direct care, collaborate with the Clinical Director, and educate families to align care with patient goals.

Qualifications

• Active, unencumbered RN license in California.
• BSN degree from NLN-accredited program or equivalent.
• Minimum 1 year of professional nursing experience within last 3 years.
• Hospice/home health experience preferred but not required.
• Proficient in computer use and EMR; CPR certified.
• Valid driver’s license and reliable transportation.

Responsibilities

• Respond to clinical referrals with compassionate, timely assessments.
• Collaborate with Clinical Director to assess patient eligibility for hospice services.
• Provide direct, hands-on nursing care to maintain comfort and quality of life.

Continuum Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

• Collaborative Care Champion: Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services. Your expertise will guide us in providing the right care, at the right time.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR
• One (1) year experience as a professional nurse within the last three (3) years.
• Hospice/home health experience preferred but not required.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$64.00 - $66.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
