Job summary

Continuum Hospice is seeking a full-time Hospice Registered Nurse Case Manager (RNCM) to join our compassionate team in Sonoma and Marin Counties. The role focuses on delivering high-quality end-of-life nursing care, coordinating services, and guiding patients and families through the hospice journey.

The RNCM will work Monday through Friday, 8 am to 4:30 pm, providing hands-on care and collaborating with the Clinical Director to determine appropriate care.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor’s degree in nursing from NLN-accredited program or equivalent.
• One year nursing experience within the last three years.

Responsibilities

• Deliver hands-on hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess eligibility of clients for hospice services.
• Provide comfort and support to patients and families, guiding care plans and goals.
• Apply nursing skills to ease pain and improve quality of life for patients.
• Educate patients and families to participate in personalized care plans.

Continuum Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor’s degree in nursing from a program accredited by the National League for Nursing; OR
  • One (1) year experience as a professional nurse within the last three (3) years.
• Hospice/home health experience preferred but not required.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification.
• Current driver’s license, vehicle insurance, and access to dependable transportation.

$64.00 - $66.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
