Job summary

Crescent Hospice in Columbia, SC is seeking a compassionate Registered Nurse Case Manager (RNCM) to provide hands-on hospice care and coordinate services across the community. This full-time role offers a Monday–Friday schedule (8:00 am–4:30 pm) with a focus on guiding patients and families through the hospice journey, ensuring comfort, dignity, and high-quality care as part of an interdisciplinary team.

• RN license in good standing.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient in EMR/clinical documentation; CPR certification if required by state.

• Collaborate with Clinical Director to assess eligibility for hospice services.
• Deliver hands-on hospice nursing care and coordinate services for patients.

Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Crescent Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$78,000 - $83,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
