Job summary

Continuum Hospice is seeking a Full Time Hospice RN Case Manager to join our nursing team delivering compassionate end-of-life care across Santa Cruz and San Jose.

The role includes hands-on nursing, care coordination, and patient/family guidance within an interdisciplinary team, with schedule Monday to Friday, 8 am to 4:30 pm. Applicants must be licensed RNs, CPR certified, and have a valid driver’s license; hospice/home health experience preferred.

Qualifications

• Current unencumbered RN license.
• Bachelor’s degree in nursing from a NLN-accredited program OR one year of nursing experience in the last three years.

Responsibilities

• Provide hands-on hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Educate and support patients and families to develop personalized care plans.

Continuum Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday - Friday, 8 am - 4:30 pm

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR
  • One (1) year experience as a professional nurse within the last three (3) years.
• Hospice/home health experience preferred but not required.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$64.00 - $66.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
