HCA Florida Woodmont Hospital is seeking an experienced Registered Nurse Case Manager to coordinate rehabilitative care and ensure optimal patient outcomes. You will work with the Rehab Program Director and Facility Case Management Director to align care plans, discharge planning, and education for patients and families.
The role requires an active RN license, 3+ years of acute care hospital nursing, and preferred certification in case management with InterQual experience.

The Case Manager (CM) ensures high-quality, patient-centered care by managing Rehabilitative care to ensure optimum outcomes. The CM provides, coordinates, and directs care specific to the needs of each Rehab patient. The CM collaborates with the Rehab Program Director and Facility Case Management Director regarding departmental functions. The CM coordinates efforts within the Rehabilitation team.
