HCA Florida Woodmont Hospital is seeking a Registered Nurse Case Manager to coordinate rehabilitative care, ensuring high-quality, patient-centered outcomes. You will partner with the Rehab Program Director and the Facility Case Management Director to steer the care plan and discharge processes.
Ideal candidates hold Florida RN licensure with a BSN preferred, and three years of acute hospital experience. Certification in case management and InterQual experience are a plus.

The Case Manager (CM) ensures high-quality, patient-centered care by managing Rehabilitative care to ensure optimum outcomes. The CM provides, coordinates, and directs care specific to the needs of each Rehab patient. The CM collaborates with the Rehab Program Director and Facility Case Management Director regarding departmental functions. The CM coordinates efforts within the Rehabilitation team.
