Job summary

BrightStar Care of Lafayette is seeking a Home Health RN Case Manager to lead planning, coordination, and delivery of skilled nursing services in patients' homes. You’ll manage your own caseload within a collaborative team dedicated to high-quality care.

The role requires an active Indiana RN license and at least 1 year of nursing experience, preferably in home health, with strong organizational and time-management skills.

Qualifications

• Active RN license in Indiana.
• At least 1 year of professional nursing experience (home health experience is a plus).
• Strong critical thinking, organization, and time management skills.
• Ability to work independently in the field.
• Reliable transportation, valid driver’s license, and auto insurance.

Responsibilities

• Deliver skilled nursing care according to physician orders and patient-specific plans.
• Conduct in-home assessments and create individualized care goals.
• Serve as a Case Manager, collaborating with interdisciplinary team members and supervising field staff.
• Provide health education and guidance to patients and their families.
• Document care accurately and efficiently using our EMR system.
• Maintain compliance with all clinical and regulatory standards.

About the Role

Job Type: Full-time

Pay: $60,000.00 - $75,000.00 per year
