Job summary

Traditional Home Health Care in Nanticoke, PA, is seeking an RN Case Manager for full-time, Monday–Friday, 8:30am–5:00pm. Join a team that provides one-on-one in-home patient care with excellent benefits and weekly pay. You will assess patients, coordinate care with physicians and family, and support a patient-centered approach in a supportive setting.

Requirements include PA RN license and BLS, a valid driver’s license, and strong documentation skills. Home health experience is preferred.

Qualifications

• Active RN license (PA) and BLS certification.
• Valid driver’s license and reliable transportation.
• Strong assessment and documentation skills.
• Acute care and/or home health experience preferred.

Responsibilities

• Assess patients and develop care plans with physicians, nursing supervisors, patients, and families.
• Provide hands-on care, administer medications, and evaluate patient progress.
• Maintain and submit timely documentation including OASIS.
• Collaborate with therapists, caregivers, and other healthcare professionals to ensure comprehensive care.
• Educate patients and families on treatment plans and self-care techniques.
• Inform patients of additional services and resources as needed.
