Job summary

Ascension is seeking a part-time Registered Nurse in Jacksonville, FL to coordinate care from admission through discharge with a patient-centered approach. You will design personalized care strategies and monitor progress in collaboration with the care team.

You must hold a Florida RN license with a Diploma in Nursing and BLS preferred. The role requires four 8-hour evening shifts per week (2:30 PM–11:00 PM) with weekend rotation, in a hospital setting.

Qualifications

• Registered Nurse license for Florida obtained before start date.
• BLS Provider preferred (AHA or ARC accepted).
• Diploma from an accredited school/college of nursing or equivalent licensure.

Responsibilities

How you’ll make an impact in this role

• Orchestrate the full interdisciplinary plan of care from admission through discharge to ensure a seamless patient experience.

• Design personalized care strategies that integrate clinical requirements with patient and family values and preferences.

• Monitor patient progress and oversee medical documentation to ensure treatment options and services align with diagnostic needs.

• Advocate for essential resources and proactively remove barriers to ensure the effective delivery of the health plan.

• Maintain continuous collaboration with supervisors and providers to optimize care implementation and post-discharge success.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.

• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

Additional Preferences:

• The ability to work four 8 hour shifts per week, 2:30 PM to 11:00 PM and rotating weekends, Saturday and Sunday
