Job summary

HCA Florida Lawnwood Hospital is seeking a Registered Nurse Case Manager (PRN) to coordinate rehabilitative care and ensure patient-centered outcomes. You will collaborate with the Rehab Program Director and the Facility Case Management Director to align departmental functions and drive care plans.

The role emphasizes patient education, planning, and discharge coordination within a multidisciplinary team to support quality outcomes.

Qualifications

  • RN with current state licensure; BSN preferred.
  • Three years’ clinical acute care hospital nursing experience required with preference for case management or 2 years of critical care.
  • Certification in case management or utilization review preferred.

Responsibilities

  • Provide program orientation to patients/families/caregivers and explain rights, benefits, and discharge criteria.
  • Coordinate patient/family education and involve them in team discussions.
  • Ensure implementation of the patient’s treatment plan and involvement throughout rehabilitation.
  • Document Discharge Planning Evaluation and psychosocial assessments and communicate needs to patients/family/caregivers.
  • Promote positive patient interactions and unparalleled patient service.

Job Summary and Qualifications

The Case Manager (CM) ensures high-quality, patient-centered care by managing Rehabilitative care to ensure optimum outcomes. The CM provides, coordinates, and directs care specific to the needs of each Rehab patient. The CM collaborates with the Rehab Program Director and Facility Case Management Director regarding departmental functions. The CM coordinates efforts within the Rehabilitation team.

What you will do in this role:

  • You will provide program orientation to patients/families/caregivers. Orientation will include the case managers role, Rehab philosophy, and continued stay and discharge criteria. Orientation will also include Medicare and insurance benefits, grievance procedures, treatment plan process, and rights and responsibilities.
  • You will act as the coordinator of patient/family/caregiver education. You will promote the participation of the patient/family/caregiver in team discussions related to plans, goals, and status. This will be conducted through Family Conferences and other interactions.
  • You will ensure the implementation of the patients treatment plan that supports the patients strengths, abilities, needs, and preferences. You will facilitate the involvement of the patient throughout the rehabilitation process.
  • You will document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments. You will communicate the social, financial, or discharge needs and preferences of the patient/family/caregiver.
  • You will assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
What qualifications you will need:
  • RN with current state licensure; BSN preferred
  • Three years’ clinical acute care (i.e. Med-Surg, Tele, PACU, ICU, CVICU etc) hospital nursing experience required with preference given to those with 2 years of case management experience or 2 years of critical care experience
  • Certification in case management or utilization review preferred
  • InterQual experience preferred
