Job summary

HCA Florida Lawnwood Hospital is seeking an RN Case Manager to support patients and families through every step of their care journey. You will coordinate services, develop care plans, and communicate with physicians, nurses, and the care team to ensure safe transitions.

The role requires an active RN license, at least three years of hospital nursing experience, and a preference for case management or utilization review certification.

Qualifications

  • RN with current state licensure; BSN preferred.
  • Three years’ clinical, hospital nursing experience required with preference given to those with 2 years of case management experience or 2 years of critical care experience.
  • Certification in case management or utilization review preferred.
  • InterQual experience preferred.

Responsibilities

  • Guide patients and families through orientation, benefits, discharge criteria, and rights.
  • Coordinate education for patients, families, and caregivers for treatment planning.
  • Develop individualized treatment plans reflecting the patient’s strengths, needs, and goals.
  • Complete psychosocial assessments and discharge planning evaluations and communicate findings across the care team.
  • Collaborate with Rehab Program Director and interdisciplinary team to ensure seamless, patient-centered care.
  • Promote HCA Healthcare’s values through every patient and family interaction.

Job Summary And Qualifications

As a Case Manager, your role will be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.

Your responsibilities will include:

  • Guiding patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
  • Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
  • Developing and implementing individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals
  • Completing psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team
  • Collaborating closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care
  • Promoting HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction

What qualifications you will need:
