Job summary

HCA Florida UCF Lake Nona Hospital in the Lake Nona area of Orlando is seeking an RN Case Manager (PRN). In this role you will support patients and families through every step of care, coordinate services, connect resources, and develop care plans that reflect each patient’s needs.

You will collaborate with physicians, nurses, and leaders to ensure safe transitions and high-quality outcomes, while upholding our values of compassion, respect, and excellence.

Qualifications

• Licensed as a Registered Nurse in Florida or able to obtain licensure prior to start.
• 1 year of RN experience in an acute care setting.
• Prior experience in Hospital Case Management.
• BSN is preferred.
• Certification in Case Management, Nursing or Utilization Review is preferred.

Responsibilities

• Guide patients and families through program orientation and explain rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting the patient’s strengths, needs, and personal recovery goals.
• Complete psychosocial assessments and discharge planning evaluations and clearly document findings and communicate needs across the care team.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care.
• Promote HCA Healthcare’s values of compassion, respect, and excellence in every patient and family interaction.

Job Summary And Qualifications

As a Case Manager, your role will be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.

Your responsibilities will include:
