The Registered Nurse 1, Care Line, provides telephonic triage and patient care advice for general and specific illnesses, health issues, counseling, advocacy, education, and referrals to ChenMed patients and families. This role involves remote clinical advice and assessments within licensure and technological capabilities. The nurse collaborates with primary caregivers and the interdisciplinary care team to ensure comprehensive patient care.

The schedule for this position is as follows each week:

• Friday 16:30 - 08:00
• Saturday 20:00 - 08:00
• Sunday 20:30 - 08:30

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

• Connect with patients via phone or video calls, interview, and record health history using computer systems.
• Provide health assessments, monitor patient health and at-home care, assist in emergencies, and promote wellness.
• Monitor vital signs such as blood oxygen, heart rate, respirations, blood pressure, and glucose, based on available technology.
• Use video chat to identify symptoms and conditions, analyze data, and determine appropriate health maintenance and outcomes.
• Collaborate with on-call providers to support clinical outcomes.
• Evaluate and document patient progress, ensuring optimal outcomes through telemedicine, using clear language.
• Enhance telehealth practices and nursing quality through effective utilization and outcome improvement.
• Utilize resources to plan and deliver safe, effective, and cost-efficient services.
• Perform other duties as assigned and modified by management.

EDUCATION AND EXPERIENCE CRITERIA:

• Associate Degree in Nursing required; Bachelor’s Degree preferred.
• Nurse Licensure Compact license required.
• Licenses for Michigan and Illinois required within 90 days of hire; additional licenses as needed within 90 days.
• Basic Life Support (BLS) certification from the American Heart Association or Red Cross required.
• At least 3 years of acute clinical nursing experience preferred.
• At least 1 year of virtual care experience preferred.
• At least 2 years in Emergency Nursing Services and Triage with older adults required.
