Existing or new National Provider Number. As wellas the following For Level 1: A Graduate of an accreditedschool of nursing, with an Associate of Science in Nursing (ASN).For Level 2: Graduate of an accredited school of nursing, Bachelor of Science in Nursing (BSN). This position is a Testing Designated Position (TDP) within the definition of the CSKT Drug Testing policy and subject to random drug testing.
