Job summary

St. Anne Home is seeking a Registered Nurse Assessment Coordinator – RNAC to manage the MDS and care management from admission to discharge, on-site and full-time.

The role emphasizes accurate MDS completion, development and monitoring of care plans, and collaboration with the IDT to tailor resident care, including certs and re-certifications.

Qualifications

• Licensed as a Registered Nurse in Pennsylvania (PA).
• Minimum of 1 year of MDS experience in care settings.
• Minimum of 1 year of Long-Term Care experience.
• Minimum of 1 year of Care Plan experience.

Responsibilities

• Complete all required sections of the MDS.
• Intensive focus on Care Plans: creation, documentation, monitoring and leading the Care Plan meetings.
• Ensure documentation is present in the medical record to support MDS coding.
• Collaborate with the IDT to assess needs and plan resident care.
• Obtain and ensure Skilled Certification/Re-Certification documentation is accurate.

Registered Nurse Assessment Coordinator- MDS Coordinator – RNAC

We are seeking to hire aRegistered Nurse Assessment Coordinator – RNAC to assist with accurate and timely completion of both the RAI and Care Management process from admission to discharge.

• $40 to $43 per hour based on experience.
• Full-time Monday through Friday.
• Fully On-site.
