Responsibilities

• Collaborates with patient, family, and healthcare team to identify mutual goals based on nursing assessment and diagnosis.
• Diagnoses, treats and evaluates responses to health problems/needs.
• Evaluates outcomes of the nursing plan of care and revises as necessary.
• Assesses, monitors, and prevents complications associated with health situations or treatment plans.
• Notifies physician of patient condition and documents actions.
• Ensures pain is assessed, treated, and reassessed.
• Delegates to other licensed/unlicensed providers based on clinical judgment.
