• Full-Time 40-Hour Traditional Schedule

• Four 10-Hour Shift Schedule
• Weekend Schedule

The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes as prescribed by physicians, complying with laws, regulations, and policies. Responsibilities include:

• Coordinating care plans with healthcare professionals and ensuring continuity of care by collaborating with physicians, agency staff, patients, families, and community resources within a Point of Care setting.
• Conducting initial and comprehensive nursing evaluations, developing care plans within state guidelines, and documenting accurately and timely in collaboration with physicians.
• Performing patient assessments at designated times and developing appropriate care plans.
• Ensuring patients meet home health eligibility and medical necessity criteria as defined by payers.

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid Driver’s License, vehicle insurance, and access to reliable transportation or public transit
