Responsibilities

The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, following physician prescriptions and complying with applicable laws, regulations, and agency policies.

• Coordinates the total plan of care with other healthcare professionals, ensuring continuity by planning and exchanging information with physicians, agency staff, patients, families, and community resources, all within a Point of Care setting.
• Conducts initial and comprehensive nursing evaluations, determines the primary focus of care, develops the plan of care within state-specific guidelines, and ensures accurate, complete, and timely documentation in collaboration with physician orders.
• Performs comprehensive assessments at designated times and develops appropriate care plans.
• Verifies patients' eligibility and medical necessity for home health services as defined by payer sources.

Education and Experience

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
