We are hiring for a full-time Registered Nurse (RN) with a 7 On / 7 Off schedule.

• Full-Time 40-Hour Traditional Schedule
• Full-Time 32-Hour Traditional Schedule
• 7 On / 7 Off Schedule
• Four 10-Hour Shift Schedule

Responsibilities

The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, following physician prescriptions and complying with laws, regulations, and agency policies.

• Coordinate care plans with healthcare professionals, ensuring continuity of care by communicating with physicians, agency staff, patients, families, and community resources, all within a Point of Care setting.
• Conduct initial and comprehensive nursing evaluations, determine primary care focus, develop care plans within state guidelines, and document accurately and timely in collaboration with physicians.
• Perform patient assessments at designated times and develop appropriate care plans.
• Ensure patients meet home health eligibility and medical necessity guidelines as defined by payers.

Education and Experience

License Requirements

• Current RN licensure in the state of practice
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to reliable transportation
