Job summary

Stellar Health Group is seeking a dedicated Registered Nurse (RN) at Woburn Rehabilitation and Nursing Center to provide high-quality, resident-centered care. You will work with an interdisciplinary team to develop individualized care plans and promote positive clinical outcomes.

Eligible candidates hold an active Massachusetts RN license and demonstrate strong assessment, communication, and teamwork skills.

Woburn Rehabilitation and Nursing Center, a dynamic and innovative healthcare facility, is seeking a dedicated and compassionate Registered Nurse (RN) to join our team of caring professionals. As an RN, you will provide high-quality, resident-centered care while collaborating with an interdisciplinary team to develop individualized care plans and promote positive clinical outcomes. Responsibilities

• Provide direct nursing care to residents, including administering medications, obtaining vital signs, and monitoring residents' conditions.
• Develop and implement individualized resident care plans in collaboration with the interdisciplinary team.
• Observe residents and promptly identify and report changes in condition.
• Communicate effectively with residents, families, physicians, and other healthcare team members to ensure coordinated care.
• Accurately document resident care, clinical observations, and interventions.
• Participate in quality improvement initiatives and support the facility's commitment to excellence in resident care.
• Follow facility policies, procedures, and applicable state and federal regulations.
• Promote resident dignity, safety, independence, and overall well-being.
• Demonstrate a commitment to the facility's mission, values, and standards of excellence.

Qualifications
