Job summary

Avera Gettysburg invites applications for a Licensed Practical Nurse to provide professional nursing care to residents in a multidisciplinary team. You will perform licensed nursing functions, assess residents, and implement care plans under RN supervision, while adhering to physician orders and facility policy.

The role requires an active LPN license, with preference for prior long-term care experience. You will collaborate with staff, document thoroughly, and participate in rounds and care

Qualifications

• Active LPN license in the state of practice.
• Long Term Care experience preferred (<1 year).
• Ability to administer medications safely and follow physician orders.

Responsibilities

• Provide licensed practical nursing care under RN direction.
• Complete resident assessments and plan of care.
• Administer medications per policy and Pyxis system.
• Participate in care planning and update plans as needed.
• Document accurately and communicate changes using SBAR.
