Job summary

Promises / Washburn House in Worcester, MA is seeking an LPN/LVN to provide professional, direct client care in a medical detoxification and residential program. You will work within a dedicated health team, following established nursing processes and standards of care.

This role requires a current LVN/LPN license, BLS/CPR certification, and at least one year of behavioral health or detox experience. You will oversee medication administration, admissions and discharge workflows, and maintain

Qualifications

  • High school diploma or equivalent required.
  • Completion of accredited LVN/LPN program required.
  • Associate’s or Bachelor’s degree preferred.
  • Current, valid LVN or LPN license.
  • Current BLS/CPR certification.
  • Minimum one year experience in behavioral health or residential detox settings.

Responsibilities

  • Oversee fulfillment of patient’s medical orders and address medical concerns.
  • Triage medical concerns and administer medications as directed by physician.
  • Monitor client self-administration of medications.
  • Screen all individuals for physical pain and refer for evaluation/management.
  • Serve as first line in medical emergencies and assist with psychiatric emergencies.
  • Perform laboratory procedures from specimen collections as authorized.
  • Record patient observations accurately in medical records.
  • Communicate with patients, physicians, nurses, and pharmacy staff.
  • Oversee admission and discharge processes including pre-screening and data collection.

JOB SUMMARY

The LPN / LVN provides professional, direct client care in a medical detoxification and residential program settings in accordance with established program procedures. This position collaborates with efforts of health team members and in accordance with the nursing process and standards of care and practice

JOB RESPONSIBILITIES
  • Oversee fulfillment of patient’s medical orders, address patient medical concerns.
  • Triage medical concerns, administer medications and injections to patients as directed by directed physician.
  • Monitors Client self-administration of medication.
  • Responsible for screening all individuals for physical pain and makes referrals for evaluation and management as appropriate.
  • Serve as first line in any medical emergency and works with the clinical team in incidences of psychiatric emergencies.
  • Perform laboratory procedures from specimen collections, as authorized.
  • Record patient observations and activities in patient records accurately and timely in accordance with nursing standards and organizational policy and procedure.
  • Communicate with patients, physicians, nurses, and pharmacy staff.
  • Oversee all aspects of admission/discharge process, including:
  1. Pre-screen of new patients for appropriateness for admission.
  2. Obtain physical assessment data and health history.
  3. Inventory and caring for patients’ personal possessions.
  4. Complete discharge paperwork.
  • Provide quality assessments to client’s medication sheets and files to determine if clients are receiving appropriate medication in a timely manner, including proper destruction of medication.
  • Follow all policies and procedures with regard to patient information protection, including alcohol-drug patient confidentiality and HIPAA.
  • Attend training on chemical dependency as required.
  • Maintains and adheres to established infection control/employee health program.
  • Maintain and demonstrates a current working knowledge of applicable clinical procedures and nursing care based on company policy and State and Federal guidelines.
  • Maintain appropriate professional boundaries with current clients, family members, referrals sources and vendors.
Education:
  • High school diplomaor equivalent required.
  • Completion ofaccreditedLVN/LPN program,required.
  • Associate’s or Bachelor’sdegree, preferred.
Certifications:
  • Current, valid LVN or LPN license.
  • Current BLS/CPR certification.
Experience:
  • Minimum one (1) year experience in behavioral health or residential detoxification program setting isrequired.
