Registered Nurse

(Full-time; 40 hours/week)

Our dedicated staff provides a full range of primary care services to all ages including medical, dental and behavioral health care, as well as care coordination and community outreach. In addition to our main facilities, we offer mobile programs and School Based Health Centers.

Our Community - Generations Family Health Center

Location: Putnam, CT

Registered Nurse Role

• Assess and identify patient needs, providing safe, evidence-based nursing interventions that promote physical and mental wellness.
• Deliver educational visits focused on chronic disease management (both mental and physical health), empowering patients to better understand and manage their conditions.
• Administer medications, including long-acting injectable medications for behavioral health treatment, in accordance with established protocols.
• Conduct telephone triage and support crisis intervention needs in collaboration with the behavioral health and medical teams.
• Provide follow-up care and outreach for patients discharged from the hospital or emergency department following a behavioral health-related event.
• Participate in interdisciplinary team meetings, contributing to the development of holistic, individualized care plans.
• Work alongside a supportive team of nurses, primary care clinicians, and behavioral health clinicians to ensure seamless coordination of care.
  
  

Qualifications

Qualifications:

• Current RN license in the State of CT
• Current Cardiopulmonary Resuscitation (CPR) / Basic Life Support (BLS)
• Bachelor of Science in Nursing (BSN), preferred
• Experience in an ambulatory and behavioral health settings, preferred
• Teaching experience, desirable
  
  

Compensation: Base salary starting at $72,800+ (compensatory salary based on experience)

• $3,000 Sign-On Bonus
