Job summary

Millennium Physician Group in Florida's Charlotte County is seeking a Registered Nurse (RN) to join our Home Health Team. You will perform comprehensive patient assessments, develop care plans, provide clinical interventions, educate patients and families, and ensure compliant documentation while visiting patients in their homes.

Responsibilities

  • Patient Assessment: Conduct comprehensive assessments of patients' physical, mental, and emotional health, including vital signs, medication administration, wound care, and disease management.
  • Care Planning: Develop individualized care plans that address patients' specific needs and goals, including medication administration, wound care, and disease management.
  • Patient Education: Educate patients and their families on disease management, medication administration, and self-care techniques to promote independence and improve health outcomes.
  • Clinical Interventions: Provide direct patient care, including wound care, medication administration, injections, and IV therapy.
  • Collaboration: Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
  • Documentation: Maintain accurate and timely documentation of patient assessments, interventions, and progress notes.
  • Compliance: Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.

Compensation Range:

$66,675.00 to $105,569.00

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
