Job summary

West Park Rehabilitation and Nursing Center in Philadelphia is seeking a dedicated Registered Nurse to join our resident-centered care team. You will deliver compassionate nursing care, assess residents, and collaborate with physicians and therapists to ensure comprehensive service.

As an integral member of our team, you will monitor vital signs, administer medications as needed, and communicate care plans to residents and families.

• Deliver high-quality patient care, assessing, and evaluating the physical and emotional needs of our residents, developing individualized care plans to address their unique needs.
• Collaborate with our interdisciplinary team, including physicians, physical therapists, occupational therapists, and social workers, to ensure seamless and efficient care delivery.
• Conduct routine assessments, monitor vital signs, and administer medications as needed, ensuring the highest level of patient safety.
• Communicate effectively with residents, families, and healthcare team members, updating them on care plans and any changes or concerns.
• Participate in quality improvement initiatives, identifying areas for improvement and implementing evidence-based practices to enhance patient outcomes.
