West Park Rehabilitation and Nursing Center, a premier 176-bed facility in Philadelphia near University City, seeks a compassionate RN Registered Nurse to join our renowned team. You will deliver patient-centered care, develop individualized care plans, and participate in interdisciplinary collaboration to support residents' health and well-being.

You will engage in routine assessments, monitor vital signs, administer medications, and communicate clearly with residents and families about care

Responsibilities

• Deliver high-quality patient care, assessing and evaluating residents' physical and emotional needs and developing individualized care plans.
• Collaborate with an interdisciplinary team including physicians, physical therapists, occupational therapists, and social workers to ensure seamless care delivery.
• Conduct routine assessments, monitor vital signs, and administer medications as needed, ensuring patient safety.
• Communicate effectively with residents, families, and healthcare team members about care plans and any changes or concerns.
• Participate in quality improvement initiatives to identify areas for improvement and implement evidence-based practices to enhance outcomes.
