• Deliver high-quality patient care, assessing and evaluating the physical and emotional needs of residents, developing individualized care plans to address their unique needs.
• Collaborate with our interdisciplinary team, including physicians, physical therapists, occupational therapists, and social workers, to ensure seamless and efficient care delivery.
• Conduct routine assessments, monitor vital signs, and administer medications as needed, ensuring the highest level of patient safety.
• Communicate effectively with residents, families, and healthcare team members, updating them on care plans and any changes or concerns.
• Participate in quality improvement initiatives, identifying areas for improvement and implementing evidence-based practices to enhance patient outcomes.
