Beacon Home Health in Paris, TN is seeking a Registered Nurse (PRN) to provide and direct nursing care in patients' homes. The RN will evaluate and plan care, coordinate with physicians, families, and community resources, and ensure compliance with state regulations and agency policies.

Key duties include initiating and following the plan of care, assessing patient needs, maintaining accurate documentation, and ensuring efficient visit routing.

Qualifications

• One year of clinical RN experience.
• Current RN license in the state where services are rendered (Tennessee).
• CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

• Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
• Make the initial/comprehensive nursing evaluation visit, determine primary focus of care, develop the plan of care within state guidelines, and ensure patients meet home health eligibility and medical necessity guidelines.
• Maintain and submit accurate documentation and clinical notes, including patient condition, goals, interventions, and responses.
• Establish and maintain an effective and efficient visit route.
• Provide high quality clinical services within the scope of practice as defined by state laws governing nursing, from admit through discharge.

POSITION OVERVIEW:

The RN provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. A few position highlights include:

• Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes. To maintain continuity of patient care, plan and exchange information with physician, agency personnel, patient, family, and community resources.

• Make the initial and/or comprehensive nursing evaluation visit, determine primary focus of care, develop the plan of care within state specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.

• Maintain and submit accurate documentation and clinical notes, including the patient’s condition, appropriate goals, interventions, and patient’s response to home health care.

• Establish and maintain an effective and efficient visit route.

• Provide high quality clinical services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, from admit through discharge.

THE MUST HAVES:

• One (1) year of clinical RN experience.

• Current RN license in the state where services are rendered.

• Current CPR certification, driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
