Beacon Home Health is seeking a Registered Nurse (RN) to join our Home Health team serving Cookeville, TN and surrounding communities. This full-time role involves delivering compassionate patient care, coordinating with physicians and families, and ensuring continuity of care across the plan of care.

The RN will travel to patient residences as part of a comprehensive home health program. Qualified candidates have an active TN RN license, CPR certification, and ability to commute to multiple

• One year of clinical RN experience.
• Current RN license in the state of service.
• Current CPR certification, driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to commute to multiple patient residences daily.

Responsabilidades

• Coordinate the total plan of care with other health care professionals involved in the care and strive to maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.
• Make the initial and/or comprehensive nursing evaluation visit, determine primary focus of care, develop the plan of care within state specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.
• Maintain and submit accurate documentation and clinical notes, including the patient’s condition, appropriate goals, interventions, and patient’s response to home health care.
• Provide high quality clinical services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, from admit through discharge.

POSITION OVERVIEW

The RN provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. A few position highlights include:

THE MUST HAVES

• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
