Job summary

Callen-Lorde Community Health Center is seeking a dedicated Full-Time Registered Nurse (RN) to support primary care teams in the Bronx. The RN will screen calls and walk-ins for urgent medical matters, triage to appropriate care, and educate patients on HIV and other health issues.

The role requires a New York State RN license and 2–3 years of acute or ambulatory care experience, with ER or triage experience preferred. A comprehensive benefits package is provided.

Qualifications

• New York State RN license required.

Responsibilities

The RN will play a crucial role in supporting primary care providers by screening patient phone calls and walk-ins for urgent medical matters, triaging patients to urgent care, and referring patients to the ER or other facilities as necessary.Key Responsibilities:As an RN, your responsibilities will include:Screen patient phone calls and walk-ins for urgent medical matters and triage patients appropriately.Refer patients to urgent care, the ER, or other facilities as needed.Provide patient education, adherence support, and case management assistance.Follow up on laboratory calls regarding alert values and ensure timely patient communication.Educate and support patients on a variety of medical issues, including HIV and HIV adherence.Provide clinical support during patient care sessions when required.Work Schedule:Callen-Lorde is open Monday through Saturday.
