Job summary

Jobtailor is seeking a skilled Home Health RN to provide patient care in Plymouth, NH, and surrounding areas. The role focuses on comprehensive nursing assessments and individualized care plans delivered in patients' homes.

You will perform wound care, catheter management, IV changes, and ostomy care while supervising LNA staff and maintaining thorough documentation. Travel and on-call rotations are expected.

Qualifications

• NH RN license is required.
• Valid driver's license and insured vehicle.
• Minimum of one year of professional nursing experience.
• Ability to travel to patient homes and work independently.
• Associate's degree or higher in nursing.

Responsibilities

• Provide skilled nursing care to patients in their homes, primarily in Plymouth and surrounding areas.
• Develop and implement individualized care plans using critical thinking and assessment skills.
• Perform wounds care, catheter management (Foley, PICC line), IV dressing changes, and ostomy/ileostomy care.
• Provide LNA supervision within the team.
• Maintain accurate and timely documentation.
• Participate in rotating weekend and on-call schedule for continuous patient support.
• Travel to patient homes in varying weather conditions.

Job description

• Utilize your critical thinking and assessment skills to develop and implement individualized care plans
• Perform a range of essential nursing skills, including wound care, Foley catheter insertion and removal, PICC line and IV dressing changes, wound vacuum dressing changes, ostomy/ileostomy management, venipunctures, and Pleurex catheter drainage
• Provide LNA supervision
• Work independently while being a valued member of our collaborative team
• Maintain accurate and timely documentation
• Participate in a rotating weekend and on-call schedule to ensure continuous patient support

Requirements

• Current New Hampshire Registered Nurse (RN) license
• Valid Driver's License and insured vehicle
