Job summary

University of TN Medical Center Home Care Services, affiliated with The Pennant Group, seeks a Registered Nurse to join our Hospice team serving Knoxville and surrounding communities. This on-the-road role requires travel to multiple patient residences daily and coordination with physicians and care teams.

The RN will perform evaluations, develop and update care plans, maintain documentation, and deliver high-quality nursing care in home settings in compliance with state laws and payer

Qualifications

• 1 year of clinical RN experience
• Current RN license in the state of TN
• Current CPR certification
• Driver's license and access to a dependable vehicle
• Must be able to commute to multiple patient residences daily

Responsibilities

• Coordinate the total plan of care with other health care professionals and families
• Perform initial and comprehensive nursing evaluations and develop the plan of care
• Ensure patients meet home health eligibility and medical necessity guidelines
• Maintain accurate documentation and clinical notes from admit to discharge
