Job summary

BAYADA Home Health Care is seeking an experienced Registered Nurse (RN) for a part-time home health role in New Jersey. You will join a multidisciplinary team delivering rehabilitative care and skilled nursing to patients in their homes across Gloucester County and surrounding areas.

The RN will perform medication administration, wound care, and clinical procedures while coordinating with clinicians to ensure optimal patient outcomes. A valid NJ RN license and ability to travel are required.

Qualifications

• Active NJ RN license is required.
• Minimum of one year of recent clinical (medical/surgical) experience.
• Home care experience is preferred but not required.
• Ability to travel to cases as assigned.

Responsibilities

• Follow a designated care plan to meet patient needs.
• Perform home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to Clinical/Client Services Manager.
• Follow up with and document doctors’ orders accurately.
• Perform client assessments as necessary.
• Coordinate case management and communication among care teams.
• Document observations and interventions on a state-of-the-art touch pad tablet.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient’s needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients’ conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors’ orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state‑of‑the‑art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in NJ.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information:

Base Pay, depending upon qualifications and subject to negotiation:
$50.00-$57.00 per point
