Job summary

Oak Park Place Madison is seeking a Registered Nurse (RN) to deliver compassionate, individualized care guided by a Plan of Care. You will monitor residents' health, document changes, and communicate updates to families and the care team.

Registered Nurse (RN) Responsibilities

The Registered Nurse (RN) responsibilities include providing thoughtful care to residents based on personalized Plane of Care, monitoring and tracking changes in the residents' conditions and needs, and proactively updating the appropriate contacts regarding the residents' plan of care, health changes, and concerns.
