Oak Park Place in Dubuque, IA is seeking a Licensed Practical Nurse (LPN) to provide compassionate care to residents under personalized care plans. You will monitor health conditions, document changes, and coordinate with family and care teams to ensure timely updates.

The Licensed Practical Nurse (LPN)

is responsible for providing thoughtful care to residents and their families based on the personalized Plan of Care, monitoring and tracking changes in conditions and needs, and proactively updating the appropriate contacts on the plan of care, health changes, and concerns.

Requirements

• Previous experience in nursing or other medical fields
• Familiarity with medical software and equipment

• Ability to thrive in a fast-paced environment
• Must have a current LPN license
