Job summary

Preferred Home Health Care & Nursing Services is seeking a Registered Nurse to deliver patient care in a home health setting. You will supervise LPNs, manage nursing visit utilization, and coordinate care with facility staff and residents.

The role requires an active Colorado RN license with at least one year of experience; home health experience is a plus, and familiarity with OASIS is preferred. A valid driver’s license and strong communication skills are essential.

Key Responsibilities Include

• Completing SOC/ROC/RECERTs, supervisory visits, and revisits based on patient needs.
• Ensuring timely and accurate documentation following agency and Medicare standards.
• Managing and providing care to patients within the RN scope of practice.
• Supervising LPNs and managing nursing visit utilization.
• Coordinating care, attending resident reviews, and maintaining communication with facility staff.
• Contributing to census growth through collaboration with marketing teams.
• Monitoring fiscal outcomes and managing patient case utilization.
• Collaborating with clinical and facility staff to optimize patient resources.
• Preparing fiscal reports and participating in care coalitions as needed.
