Open Sky Community Services in Massachusetts is hiring a Registered Nurse for community-based care outside hospital settings. You will work within residential and community environments, focusing on recovery-based care and long-term relationships with individuals diagnosed with mental illness or dual diagnosis.

You will perform physical assessments, coordinate with physicians, supervise medication administration, and provide health education.

Qualifications

• Registered Nurse license in Massachusetts is required.
• Valid driver’s license with acceptable driving history required.
• Excellent assessment, documentation, and verbal communication skills required.
• Experience with adults with mental health and psychiatric disorders preferred.

Responsibilities

• Provide medical training and consultation to programs on health issues, procedures and wellness practices.
• Educate individuals on health and disease processes and reinforce wellness and preventive care.

We provide opportunities for nurses that are beyond the four walls of an institutional setting, as you work within residential and community settings, focusing on recovery-based care for people diagnosed mental illness or dual diagnosis.

Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, or in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky's talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to individuals served.

The Registered Nurse collaborates with the integrated team to complete thorough physical assessment of health needs, referrals to appropriate community physicians/health care providers, and monitor medical/psychiatric status of individuals served by observing and interviewing individuals, examining documents, and making recommendations.

Other Key Responsibilities

• Provide training and consultation to programs regarding medical issues, procedures and techniques, and health and wellness practices.
• Assess and provide individualized education related to person's health and disease processes; reinforce wellness and preventative health measures.
• Review clinical records on a regular basis to ensure adequate services and supports are provided, including any applicable medical interventions, tests, or preventative care as necessary.
• Provide oversight to the Medication Administration Program and ensure compliance with applicable regulations.
