Millennium Physician Group in Lee County, Florida seeks an experienced Registered Nurse (RN) to join our Home Health team. You will perform comprehensive patient assessments, develop care plans, educate patients and families, administer medications, and collaborate with physicians and therapists to ensure safe, effective home-based care.

Requirements include Florida RN licensure and at least 1 year of hospital or SNF experience.

Qualifications

• Florida RN licensure required.
• Minimum 1 year of hospital or skilled nursing facility experience.
• Strong clinical skills and patient-centered approach.
• Excellent communication and documentation abilities.
• Reliable transportation for patient home visits.

Responsabilités

• Conduct comprehensive patient assessments (vital signs, meds, wound care).
• Develop individualized care plans and implement patient care.
• Educate patients and families on disease management and self-care.
• Provide direct patient care including injections and IV therapy as prescribed.
