Job summary

Lacey Post Acute & Rehabilitation in Lacey, WA is seeking Registered Nurses (RNs) for full-time, part-time or per diem roles. You will provide nursing care, coordinate rehab, administer medications, educate families and residents, and ensure high-quality resident care with a team-focused approach.

Qualifications

  • Active state RN license is required and CPR/BLS certification.
  • Experience in Long Term Care or Skilled Nursing preferred.
  • Excellent technical, assessment and documentation skills.
  • Strong analytical and problem-solving skills.
  • Ability to lift 25+ pounds and stand/bend/walk for extended periods.
  • Team player with strong communication and passion for senior care.

Responsibilities

  • Provide nursing care to meet resident needs per standards and physician orders.
  • Plan nursing interventions based on individual needs and monitor implementation.
  • Evaluate care interventions, identify problems and adapt accordingly.
  • Conduct family and resident education.
  • Administer medications and treatments per center policy.
  • Transcribe orders and chart progress reflecting resident response.
  • Identify and report changes in condition to supervisor, physician and family.
  • Follow skin management protocols and document skin changes.
  • Order and report diagnostic tests timely.
  • Coordinate rehabilitation with therapists to meet needs.
  • Ensure CNA documentation and assignments are completed.

Rate of pay: $42.00 to $48.02 (plus Shift Diffs as applicable)

What You’ll Be Doing:

  • Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines

  • Plan nursing interventions for each resident based on individual needs and monitor the implementation

  • Evaluate the effectiveness of care interventions, identify problems and trends and develop alternative interventions.

  • Conduct family and resident education

  • Administer medications and treatments timely and according to center policy

  • Transcribe physician’s orders and chart progress notes in an informative, factual manner that reflects the care administered as well as the resident’s response

  • Identify and report changes in condition to supervisor, physician and family

  • Accurately identify skin changes and follow skin management protocols

  • Order and report diagnostic tests timely

  • Follow Pharmacy policy and procedures for ordering and delivering medications.

  • Coordinate resident’s rehabilitation program with therapists to ensure rehabilitation needs are met

  • Monitors that CNA documentation is completed daily

  • Ensures CNA assignments are completed on time

  • Perform additional nursing care responsibilities as required and based on State Nurse Practice Act.

Requirements:

  • Active state RN license

  • CPR/BLS Certification

  • Experience in Long Term Care or Skilled Nursing preferred
