Job summary

Addus HomeCare, Inc. is seeking a Full-Time Registered Nurse (RN) to provide one-on-one skilled nursing in patients' homes in Jackson, TN, and surrounding areas. The role focuses on assessment, care planning, and coordination with a clinical team to promote recovery and independence.

You will perform admissions processing, plan and evaluate care, and support families. Travel, documentation, and adherence to licensure are essential, with comprehensive benefits available.

Qualifications

  • Graduate from an accredited registered nursing program and licensed in the state.
  • At least 1 year clinical experience, home health preferred.
  • CPR certification maintained; reliable transportation and valid driver's license.

Responsibilities

  • Provide visits as assigned, perform nursing assessments, admissions processing and care planning.
  • Gather data from patient and family and coordinate with interdisciplinary team.
  • Communicate with physician and agencies to coordinate optimal care.
  • Update plan of care and perform nursing procedures as directed.
  • Document patient records timely and maintain charts.

Jackson TN, and surrounding areas

Schedule: Regular weekday patient visits with participation in a rotating on-call schedule, including evenings and weekends. Daily hours vary based on patient needs, travel, and documentation.

RN What You’ll Do:
  • Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
  • Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
  • Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
  • Provide visits to assess the patient's needs and update the plan of care
  • Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
  • Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
  • Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
Qualifications:
  • Registered Nurse
    • Graduate from an accredited registered nursing program and licensed in the state
    • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
    • Home health experience preferred
    • Possess and maintain valid CPR certification.
    • Must have reliable transportation, current driver's license and appropriate automobile insurance
