Job summary

Tennessee Quality Care in Dover, TN is seeking a PRN Registered Nurse (RN) to provide one-on-one home health visits in Dover and surrounding areas. Schedule includes regular weekday patient visits with a rotating on-call requirement that may include evenings and weekends as patient needs require.

You will assess and plan care, collaborate with the Clinical Services Manager and interdisciplinary team, and maintain accurate records.

Qualifications

• Graduate from an accredited registered nursing program and licensed in the state.
• At least 1 year clinical experience preferred, especially in home health.
• CPR certification required.

Responsibilities

• Provide home visits as assigned by Clinical Services Manager, including assessment, planning, implementation, and evaluation of the nursing process.
• Coordinate care with interdisciplinary team and attending physician.
• Maintain accurate patient records and care plans, and ensure timely documentation.

RN What Youll Do

• Provide visits as assigned by the Clinical Services Manager including the assessment determination of eligibility admissions processing planning implementation and evaluation phases of the nursing process
• Assignments will vary depending on licensure
• Obtain data and assess necessary information from patient physical psychological social and spiritual factors that may impact patient and familys needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patients needs and update the plan of care
• Perform therapeutic preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family as applicable to licensure
• Maintain accurate patient records charts progress notes reports and prepare care plans timely

Qualifications

• Registered Nurse
• Graduate from an accredited registered nursing program and licensed in the state
• At least one 1 year clinical practical nursing experience preferably in intensive care rehabilitation or medical surgical nursing preferred
• Home health experience preferred
• Possess and maintain valid CPR certification
• Must have reliable transportation current drivers license and appropriate automobile insurance
